色花堂's mandatory immunization and TB testing, required for every student, ensures a healthy community.

Requirements, Recommendations and Deadlines

Required Immunizations

  • Measles-Mumps-Rubella (MMR): Two doses given on or after first birthday or copy of blood test showing immunity with positive IgG titers to Measles (rubeola), Rubella and Mumps.
  • Tetanus-Diphtheria-Pertussis (Tdap): One dose given at age 11 or older.
  • Meningococcal (Meningitis) ACWY (Menveo, MenQuadfi, or Menactra): given 鈮 16 years of age is required by Maryland law for students living in on-campus housing. This vaccine can be waived by completing the waiver on the housing contract.
  • Tuberculosis (TB) Exposure Risk Screening Questionnaire: Must be completed in the . If any questions are answered 鈥淵ES鈥, you are required to have a QuantiFERON Gold blood test.

Submission Deadlines

All immunization documentation must be submitted upon enrollment. 

  • Aug. 15 for fall enrollment
  • Jan. 25 for spring enrollment

Failure to submit required documentation by the deadline may result in a $50 late fee and delays in course registration.

  • Meningitis B vaccine (Trumenba or Bexsero)
  • Varicella (chicken pox) vaccine
  • Hepatitis B vaccine
  • HPV vaccine (Gardasil 9)
  • Up to date COVID-19 vaccine
  • Seasonal influenza (flu) vaccine

Additional Requirements

You must come to the Health Center upon arrival at 色花堂 for a TB testing visit. A TB test is required, regardless of prior BCG vaccination. A chest X-ray alone is not acceptable. If you have a TB blood test performed within 6 months of arriving to 色花堂, bring this documentation with you.

If the blood test was positive, bring the official results in English. If you have ever been treated for a positive TB test or active tuberculosis, bring documentation of drugs and duration of treatment. You will not be allowed to remain in classes if you do not complete this requirement.

The College of Health Professions and other programs may have additional immunization and tuberculosis screening requirements. Please contact your department directly.

How to Submit Documentation

Collect Your Documents

You will need a copy of your official immunization records. If you are not able to get a form from your primary care provider, download the immunizations form to be filled out by your medical provider.

Sign in to the . Use your 色花堂 log-in.

Enter Your Immunizations and Complete Your Questionnaire

To enter your immunizations, click on the Immunization Requirements link at the top of the page to enter the dates of your immunizations. You will also need to upload your official document.

After you enter and save your immunizations, return to the home page and to the Tuberculosis Exposure Risk Screening Questionnaire. 

  • On the left side bar, click on "Forms." This may not appear like a typical hyperlink, but if you hover over 鈥淔orms鈥 and click, this will take you to the questionnaire. 
  • Click on "Required: Tuberculosis Exposure Risk Screening Questionnaire".
  • Answer all the questions and click on Submit Final. 

Immunization Review Process

The Health Center team reviews all submissions for required immunizations.

Reviews may take between 5 to 7 business days before they are registered to student accounts. If you have questions about your immunization compliance status after this time period has elapsed, please contact us at 410-704-2466.

Getting a vaccine on campus

  • The Health Center offers MMR, Tdap, Meningitis ACWY, Meningitis B, seasonal influenza, HPV, Hepatitis A, and Hepatitis B. Please call 410-704-2466 to schedule.
  • In the fall, 色花堂 partners with Safeway to offer multiple flu and COVID vaccine clinics at convenient locations on campus.  

FAQ: Immunizations

No, you do not have to. We will accept immunization records if they are legible, and the record contains your name and date of birth. You can have your health provider complete our immunization form (PDF), but this is not required if you submit a valid copy of your immunization records.

Yes, you will need to also submit proof of the immunizations by submitting a copy of your immunization records on your .

  • The Health Center offers all vaccines that are required. Please call 410-704-2466 to schedule.

  • MMR titers: A titer is a blood test that can prove immunity to disease. If you cannot find records of your MMR vaccines, your medical provider can perform titers to verify positive immunity. You must provide a copy of the lab report for verification. You can also schedule a lab visit at the Health Center to have this testing done. Please call 410-704-2466 to schedule. If the titers show that you are not immune, then you will need to get the MMR vaccine.
  • Ask parents or other caregivers if they have records of your childhood immunizations.
  • Check with your doctor or public health clinic to get a copy of your immunization records.
  • Contact your state's health department. Some states have registries (Immunization Information Systems) that include adult vaccines. The State of Maryland has an online system called MyIR Mobile, where you can your immunization records.
  • Check with your high school and/or previous college health services for dates of any immunizations. 
  • Check with previous employers (including the military) that may have required immunizations.

FAQ: Vaccine and TB Screening Requirements

Yes, 色花堂 requires one dose of Tdap given at age 11 or older. Td protects you against tetanus and diphtheria, while Tdap protects you against pertussis (whooping cough) as well as tetanus and diphtheria.

No, 色花堂 will NOT accept the PPD skin test in lieu of the QuantiFERON Gold test.

If your QuantiFERON Gold Blood test result is positive, you will need to have a chest x-ray to make sure you do not have active tuberculosis. You must attach your x-ray report (in English). You can arrange this through your healthcare provider. You can also schedule an appointment at the Health Center with a provider to discuss the QuantiFERON Gold test result, get an order for a chest x-ray, and discuss treatment options for latent tuberculosis.
